the 'not found' message refers to the fact that the ApacheMonitor could not fetch the server-status page from the server being monitored.
on your apache enable status support, or better still, enable support for extendedstatus - there are guidelines here:-

as a rule, mod_status is built-in to most apaches. all you have to do is modify a config file.
once you can access the server-status url in a web browser ok, then the ApacheMonitor will be able to monitor it also.
hint: you do not have to map the server-status page to all domains or to the default url of server-status.
while using the default url and mapping to all domains will likely make it easier to successfully set up ApacheMonitor, conversely, mapping the status page to a single domain, ip number or port (possibly not accessible to the outside), and/or using a different url from the default will provide more secrecy/security but may make it trickier to initially set up ApacheMonitor.
btw, there was a recent bug in ApacheMonitor whereby it wouldn't use a non-default port for the status url - i do not know if that has been fixed in current releases yet?
